That’s why Sep 19, Service Coordination Appreciation Day, is so important to us.
Our Service Coordinators work inside senior affordable housing complexes. For residents, it means having someone to turn to when you don’t know where to go for help. Someone who understands aging issues like isolation and the importance of social connections. Service Coordinators not only provide resources and prevent crises, they also host social activities and engage with outside groups to bring services to residents directly.
Last year, Service Coordinators assisted 500 older adults living in affordable housing.
For one resident, Service Coordination made it possible to live in the home he wanted. After suffering several strokes and a heart attack, he spent months in rehabilitation and nursing facilities before finally returning to his apartment. But his life had changed dramatically. He was now living with significant changes to his eyesight, mobility, and memory.
Without the right support, living on his own might not have been possible. His Service Coordinator helped arrange therapy, nursing visits, transportation, medication support and other services. She continues to help him manage appointments, benefits and the challenges that come with his changing health.
He wanted to be home. Service Coordination helped make that possible.
And he’s not alone. Last year, nearly half of the residents in the affordable housing communities we serve, turned to a Service Coordinator for assistance.
Growing older may mean we have more problems, but it doesn’t take away our desire for independence and the simple joys of life. Our Service Coordinators know and support this.
Today we celebrate them.
