Service Coordination is provided at several low-income housing properties to help older adults remain independent and engaged in the community. Family Eldercare's Service Coordinators work on-site to help residents identify and acquire the services they need to remain self reliant.
Service Coordinators offer assistance on all levels, including helping residents address basic needs, obtain health care services and secure benefits. They also organize educational and social programs and volunteer activities.
Service Coordinators are especially important in helping residents maintain their health and housing during a crisis, and when they return from hospitalizations.

The Caleb 5K Fun Run, held in honor of Caleb Sterling Koke, benefits 5 non-profit organizations who are dedicated to meeting the needs are those who are less fortunate in the local and global community.
We are honored to have been selected as a beneficiary of this fun, annual event hosted at Shoreline Church. Early bird registration has already begun (ends May 6!), and regular registration will continue through the event day. We hope that you will register and spread the word to friends and family.
